What I will learn?

  • Define medication errors and describe common causes, types, and strategies to prevent them within the medication-use process.
  • Identify and classify adverse drug reactions (ADRs) and explain appropriate steps for monitoring, reporting, and managing these events to ensure patient safety.
  • Recognize high-alert medications and apply recommended safety precautions to minimize the risk of serious harm associated with their use.
  • Distinguish between look-alike/sound-alike (LASA) medications and implement effective measures to reduce selection and dispensing errors related to similar names or packaging.
  • Apply prohibited or unsafe abbreviation policies to ensure clear, accurate, and standardized medication documentation and communication
  • Identify potential drug–drug interactions and use appropriate tools and resources to assess, prevent, and manage clinically significant interactions.
  • Demonstrate proper medication storage practices, including temperature control, light protection, segregation of high-risk items, and maintenance of storage standards.
  • Perform accurate medication reconciliation at transitions of care to identify discrepancies, ensure continuity of therapy, and prevent medication-related problems.
