for my personal experiences with difficult patients, don't try to upstand with them in fighting, just keep calm and try to explain what your are doing or clear any misunderstandings. I they escalate up, just use your right and refer them to the relevant authorities to deal with the situation in a formal way. But ever never try to lose your temper because any thing can be use against you, and always document everything to preserve your and the patients rights. And finally, it is important to be neutral with the patient and the coopatient and not keep this situation to affects your management or care. If is a family member, just go away and keep him or her until calm down to clear things in rational and emotionless way.
